Enter your last name. Enter the last four digits of your USC ID number as your Login ID. Remember that your USC ID begins with an alphabetic character. If you do not know your USC ID, you can login to Self Service Carolina to get it.

Click on [OK].

This will bring you to a screen that has your name at the top and lists the College of Nursing courses in which you are currently enrolled. If the name at the top of the page is not yours, click on the button at the top right corner of the page to log out. If the list of courses you see is not an accurate reflection of your College of Nursing courses, please contact Cheryl Nelson at (803) 777-8777 or E-mail: cheryl.nelson@sc.edu.

Choose the course you want to evaluate and click on the box that says [Click to complete].

This will bring you to the evaluation questionnaire for that course and instructor. Click on your responses and enter any additional comments. Teacher evaluation questions are first, followed by Course evaluation questions.

At the end of the questionnaire, click on the [Submit] button.

This will take you back to the page with your courses listed. You can continue to complete evaluations or log out and return later to complete any evaluations that have not been entered. Once you have submitted an evaluation, you cannot go back into the evaluation for that teacher/course.

• The evaluations are completely anonymous and confidential. Results in group format are shared with individual faculty and administrators at the College of Nursing only after final grades have been submitted.
